Request for Sheriff to Serve Restraining Order Forms
If you have a restraining order and would like to ask the Sheriff’s Office to serve, complete the form listed below:
If your case is/will be filed in Santa Clara County AND the address for service is also in Santa Clara County, you may submit SER-001 to the court along with your Restraining Order forms. They will forward your paperwork to the Santa Clara County Sheriff’s Office for service.
If the address for service is outside of Santa Clara County, you must submit form SER-001 and a copy of your filed Restraining Order Forms to the Sheriff's Office in the county where service will occur.
