2. How to get a Civil Harassment Restraining Order form
    

   To get a Civil Harassment Restraining Order, you must fill out the correct forms. You can get them from:

   • Bookstores and print shops that have legal forms

   • Request for Civil Harassment Restraining Orders (Form CH-100 )
   • Confidential CLETS Information (Form CLETS-001 )
   • Request for Sheriff to Service and Sheriff's Fee Statement (Local form FM-1041 )
   • Fee Waiver - if this applies (Forms FW-001 AND FW-003 )

3. How to fill out and file the Restraining Order form
    

   Fill out your forms and give them to the Court Specialist in the Calendar Office (Room 104) at the Downtown Superior Court. A judge will look at your forms within 24 hours and let you know if you will get a temporary restraining order or not.

   A court hearing will be set in about 3 weeks so the judge can decide if the orders will continue for up to 5 years. Make sure to come to this court hearing.

4. Fees and Waivers

   The cost to file a Civil Harassment Restraining Order is on the local fee schedule - look under Requests for Protective Orders and find the fee listed for "Petition to prevent civil harassment (other) and response."

   As indicated on the fee schedule, there is no filing fee if the person you are trying to keep away has been physically violent or threatened to be violent to you.
